To determine which services your loved one can benefit from, an Initial Assessment is completed by one of our consultants. This assessment is usually conducted with your loved one in their home.
Our fees are assessed according to services performed and are individualized according to a client's needs. Fees are always discussed and agreed upon prior to services being performed.
Some areas that may be included in an assessment are:
Medical- A review of current medical conditions, including medications, will be performed. Recommendations may include referrals to specialists. We can accompany your loved one to doctor's appointments, providing an additional communication link. We also provide assessments of need following hospital stays.
Social - An evaluation of social needs and interests is completed. Regular home visits are available to monitor a client's well being. We also provide a variety of skills training encouraging stimulation, such as computer lessons.
Mental and Emotional - Psychological evaluations including competency, cognitive loss, anxiety and depression can be performed in order to determine available options.
Home - An evaluation to determine which in-home services a client may benefit from is conducted. Just a few of the services that have helped others retain their independence include housekeeping, transportaion to appointments, companionship, grocery shopping, meal preparation, home repairs and pet care.
Safety and Emergency Planning - An assessment to increase home safety and plan for weather related emergencies such as hurricanes, can be very beneficial. In the event of a storm, being prepared can having a completed plan can make a stressful sitation safer and more manageable for seniors. Home safety such as bathroom safety grab bars and height and accessibility of frequently used items can eliminate unnecessary injuries.
